Ohio Home Care Waiver

The Ohio Home Care Waiver allows those with physical disabilities and unstable medical conditions to receive care in their homes instead of nursing facilities, hospitals or rehabilitation centers. This important service allows these individuals to have more control over their lives and remain active participants in their communities.

Adults with disabilities age 18-59 may be referred to our program by a physician or medical provider. One of our nursing supervisors will meet with them to conduct a needs assessment. If the individual meets the eligibility requirements for the Ohio Home Care Waiver and the financial eligibility for Medicaid, they can be enrolled in the program. One of our skilled case managers will be assigned to help develop a care plan and coordinate services.
